A blue border surrounds each side of the card. The front has a blue 'wheelchair' symbol with the text "For your attention. The holder of this card is assisting a disabled person to access your goods/services. Signed on reverse.".

The reverse of the card is also pictured here. It has a space for a photograph of the carer / PA.

On the right hand side is the text "I have an impairment and authorise the person shown here to act or speak on my behalf". This person can then sign or make their mark below in the space provided. The name or signature of the PA can be written below with the text 'Personal Care Assistant' beneath.
